The Labrador Retriever is a wonderful breed and we want to make sure that your new canine family member lives up to all your expectations. Like people, each puppy is an individual, with its own personality. This questionnaire will help us select the most suitable puppy for you while allowing you to clarify your thoughts about dog ownership. It also lets you see what things are important to us in placing our precious puppies. Each and every puppy that graces our home is special to us, and a member of the family. No puppy will ever go to anything but a private home with a new family to love.

How did you come to contact us?

How did you become interested in the breed?

Do you own or have owned a Labrador Retriever in the past?

If so, where are they now?

Do you own any other pets? If yes, please list types and ages:

  • What are your expectations for your dog?
  • Check the answers that apply. It is okay to mark more than one category:
  • Personal companion    Hunting companion Show/breeding     Obedience competition
  • Tracking/search and rescue Animal Assisted Therapy  Agility competition
  • Other:
  •
  • Do you have a preference for a male or a female puppy? Would you take a puppy of the opposite sex should your first choice not be available?
  • Tell us a little bit about the makeup of your household. Spouse, partner or roommates?
  • Children and their ages? (Labradors are especially good with children, but must be taught manners)

  • Who will be responsible for the care and training of your pet?

  • What family activities would you include your dog in?
  • Do you live in a house or apartment? How large is your yard and what type of fencing do you have? Please describe, including height:

  • Labrador Retrievers are athletic dogs that require regular exercise. This could mean daily walks on lead, swimming, field training, hiking,
  • running, games of fetch, or playing with other dogs in a secure area.
  • Have you thought about how you will handle your dog's exercise needs? Please describe:

  • How many hours each day during the "work week" will your dog be left alone?
  • Do you have a secure place to leave your pet while you are away from home?
  • Where will your pet sleep at night?
  • NOTE: Most Labrador Retrievers do not do well confined alone in a pen or yard. A bored Labrador is a destructive Labrador.

  • If travel plans took you away from home, what arrangements would you make for your dog?
  • Boarding kennel? Pet sitter? Neighbors or relatives? Take your dog with you?

  • Training and proper socialization are important to a puppy's development into a secure and happy companion.
  • Can you devote the time needed during the critical first few months to teach manners and expose your puppy to many new experiences?
  • Do you have any ideas about where you would go for obedience training or socialization?
  • Please list any references you may have from other breeders concerning pet ownership.

Finally, you must be aware that although we take great care in screening our breeding stock, there is no 100% guarantee that your puppy will be free of health problems

Even two parents who are free of hereditary diseases can produce a health problem occasionally.

Screening does reduce the incidence of problems and all breeding stock has preliminary or permanent OFA clearances.

Check this box to indicate that you have read and understand the above statement, your application will not be accepted without acknowledgement.
